Required Attachments for CCHD Grant Applications

Below you will find the required attachments for all National CCHD grant applications.

Local CCHD grant application requirements vary by diocese.   Please check with your Diocesan Director

If you have received an email from CCHD that provides you with a link to the online application, you must complete the following attachments and submit them with your application.

If you have not received an email from CCHD inviting you to apply, you should not fill out these forms.

The following attachments must be included with all applications:

  1. Application budget form (Excel)
  2. Board member profiles form (Word.doc)
  3. Complete board meeting minutes for the past six months (board meeting agendas will not be accepted—minutes must at minimum include who is present at the meeting, a financial update, important discussion items and outcomes)
  4. Key staff resumes
  5. Press clippings from the last 12 months (no more than two)
  6. Institutional or coalition membership list (Word.doc), if applicable

You must use CCHD forms for the budget, board member profiles, and institutional/coalition membership.

The following attachment must also be included if you are submitting an Interim Report and Refund Application:

INSTRUCTIONS FOR ATTACHING DOCUMENTS TO YOUR APPLICATION

Download each of the required form attachments by clicking on the corresponding link. Save a copy to your computer. Next fill out and complete the form. Save it once more with a different name. e.g. [file-name-date].

Proceed to follow the instructions in the online application to attach each file and submit them to CCHD with your application.

All required attachments must be attached to your application prior to submitting your application.
